  1. The GSV destruction impairs the skin drainage and the capillary pressure increases so forcing and dilating smaller veins up to matting, spider veins and varicose veins. The consrvation of the GSV by hemodynamic means, reduces dramatiacally (paroxically?) the recurrence rate. That's proved by CHIVAvs vs Stripping and Endovenous procedures studies and RCTs and Cochrane review. The adventage of CHIVA is best results, less recurrence and GSV saving for future arterial and coronary by-pass. The obstacle is intellectual ( need ofdeep knowledge of venous hemodynamics ) and no profit for usual congres sponsors because it needs just few local anestetics and suture.
    GSV preservation is not only a scientific chalenge, but also an ethical. (Some days ago a vasular surgeao who destroys hundreds of GSV asked to be personally treated by CHIVA procedure...)
    GSV CAN BE PRESERVED and the Varicose veins properly treated with less recurrences
    A- Conservative methods as CHIVA provide 50% less recurrences than stripping and less complicatons..
